The Effect of Preoperative Joint Flexibility and Grip Strength on Shoulder Function after Rotator Cuff Repair

Purpose:Shoulder pain and impaired shoulder function significantly affect a patient’s satisfaction with surgical outcomes. Several studies have suggested the decreased flexibility of the hip joint and hand grip strength is associated with developing the shoulder problems and function. This study aims to evaluate the influence of preoperative hip joint flexibility and hand grip strength on the recovery of shoulder function after rotator cuff repair. Methods:66patients diagnosed with a rotator cuff tear underwent arthroscopic rotator cuff repair performed by an orthopedic surgeon. Preoperative measurements included the straight leg raise (SLR) test, the sit-and-reach test, and Hand Grip Strength (HGS). Shoulder function measured by a questionnaire were evaluated pre and postoperatively at 6 months, 1 year, 2 years. Results: American Shoulder and Elbow Surgeons scores showed statistically significant differences in SLR (p = 0.042) and HGS (p = 0.027) after surgery for 1 year, and SLR (p = 0.044) and HGS (p = 0.006) at 2 years postoperatively. Additionally, SLR (p = 0.023) and HGS (p = 0.001) also showed significant changes in the Constant score at 1 year after surgery. Only the HGS (p = 0.003) changed the Constant score at 2 years post-surgery. Conclusion:This study shows that the importance of evaluation on preoperative hip joint flexibility and HGS for predicting the recovery of shoulder function following arthroscopic rotator cuff repair. Improved flexibility of the hip joint and greater HGS appear to facilitate better postoperative outcomes in shoulder function, as reflected in higher ASES and Constant scores.
