Non-suicidal self-injury is a significant mental health problem among the adolescents globally, frequently cooccurring with numerous psychiatric symptoms, highlighting the urgent need for early intervention and targeted prevention strategies. This study aims to validate the psychometric properties of the Repetitive Non-suicidal Self-injury Questionnaire (R-NSSI-Q) in the South Korean context. The psychometric evaluation included exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) to assess the factorial structure. The internal consistency, concurrent validity, test-retest reliability, predictive validity, and optimal cut-off score for differentiating between occasional and repetitive NSSI groups were also examined. The EFA and CFA results (CFI = .995, TLI = .994, RMSEA = .040) demonstrated a superior fit for the unifactorial model, confirming its structural validity. The K-R-NSSI-Q exhibited strong internal consistency (Cronbach's alpha = .918) and test-retest reliability (r = .795). Correlation analysis showed significant associations between the K-R-NSSI-Q and NSSI-related mental health variables, confirming concurrent and predictive validity. Furthermore, the K-R-NSSI-Q effectively discriminated between occasional and repetitive NSSI groups, suggesting an optimal cut-off score of 17.5. Given these results, the implications for adapting this scale to examine the characteristics of adolescent NSSI and screen high-risk groups are discussed.
